Injury Management Guide

Addressing injuries promptly and effectively is crucial to ensure a swift and complete recovery. The Cioffredi & Associates Physical Therapy team has put together a comprehensive guide on what to do when you get an injury, based on the latest research in sports medicine and physical therapy.

First Steps: PEACE & LOVE Injury Protocol

What should you do when you injure yourself? Well, your first move is to follow the PEACE & LOVE protocol, which is now favored over the traditional RICE (Rest, Ice, Compress, Elevate) method for acute, soft-tissue injuries. This new method provides an updated approach to promote long-term healing and rehabilitation providing guidance not just for the immediate actions you should take, but also the next steps building back up to activity. It de-emphasizes rest in favor of protection and then progressive loading and exercise which aligns with current evidence showing that early, controlled movement can enhance tissue healing and functional recovery. In addition, where RICE focused on reducing inflammation, recent research supports the benefits of avoiding anti-inflammatory medications and icing during the acute phase of injury, as inflammation is a critical part of tissue repair. Bottom line, these new changes ultimately promote better long-term outcomes after you sustain an injury:

PEACE (Immediate Care):

  • P (Protect): Avoid activities and movements that increase pain during the first few days post-injury.
  • E (Elevate): Elevate the injured area above the level of the heart to improve blood flow to the injured area and aid in healing.
  • A (Avoid Anti-inflammatories): Avoid taking anti-inflammatory medications as they can interfere with tissue healing.
  • C (Compress): Use elastic bandages or taping to limit local swelling and prevent further bleeding within the injured tissues.
  • E (Educate): Understand your injury and the importance of managing it effectively to ensure proper healing. Learn more with our Injury Finder tool.

LOVE (Subsequent Management):

  • L (Load): Gradually resume normal activities and load the injured area with appropriate movements to promote healing.
  • O (Optimism): Maintain a positive mindset to aid recovery; psychological factors play a significant role in physical healing.
  • V (Vascularization): Engage in pain-free cardiovascular activities to boost blood flow and aid tissue repair.
  • E (Exercise): Incorporate pain-free exercises to restore mobility, strength, and proprioception.

When to Seek Professional Help

That’s all well and good, but how do you know when you need to move beyond self-care methods and get professional help? While minor injuries can often be managed with self-care, certain situations require professional intervention:
  • Persistent Pain: Nagging pains that don’t improve after a few days.
  • Limited Mobility: Significant discomfort or fear of movement.
  • Swelling and Bruising: Persistent swelling or bruising that doesn’t subside.
  • Recurrent Issues: Recurring injuries or chronic pain.
Early professional intervention can prevent minor issues from becoming major problems and ensure a more efficient recovery process. By following these guidelines and seeking professional help when necessary, you can effectively manage your injury and return to your sport even stronger than before.
