Insurance, Self-Pay, and HCFA Forms — Frequently Asked Questions (FAQ)
Do you accept insurance?
We work with a limited number of insurance plans and are out of network with many commercial and Medicaid plans. When we are out of network, patients may choose to receive care on a self-pay basis.
Our front office team is happy to help you understand your options before scheduling.
What does “Out-of-Network” (OON) mean?
Being out of network means we do not have a contract with your insurance company. As a result:
- We do not bill your insurance directly
- Payment is made by you at the time of service
Some insurance plans offer out-of-network benefits, which may allow you to seek reimbursement directly from your insurance company.
Will you submit claims to my insurance if you are out of network?
No. We do not submit claims to out-of-network commercial insurance plans.
If you have out-of-network benefits, we can provide you with a HCFA form so you may submit it to your insurance company for possible reimbursement.
What is a HCFA form?
A HCFA form is a standardized insurance claim form used by outpatient providers like physical therapy offices. It includes information insurance companies use to review claims, such as:
- Dates of service
- Type of treatment provided
- Diagnosis codes
- Provider information
This information is not included on standard payment receipts or patient statements, which are designed to show payments—not insurance claim details.
Will my insurance reimburse me if I submit a HCFA form?
Reimbursement is not guaranteed. Your insurance company determines:
- Whether reimbursement is available
- How much (if any) they will reimburse
- What documentation is required
If reimbursement is approved, payment would come directly to you, not to our office.
When do I pay for self-pay visits?
All self-pay visits are paid at or before the time of service.
If you receive reimbursement from your insurance company later, that reimbursement is separate and paid directly to you.
When will my HCFA form be available?
HCFA forms can only be generated after your therapist completes documentation and charges are finalized.
Typically:
- The form is available the next business day, or
- It can be picked up at your next visit
It may not be available immediately following your appointment.
Will the HCFA form show what I paid?
No. HCFA forms reflect services provided, not payments made. If your insurance company requires proof of payment, you may also need:
- Payment receipts
- Visit statements
We’re happy to provide this information upon request.
How will I know my options before scheduling?
When you call to schedule an initial evaluation, our front office team will:
- Review whether we are in network or out of network with your plan
- Explain self-pay options and pricing
- Let you know if a HCFA form can be provided for possible reimbursement
Our goal is to help you make an informed decision before starting your care.
Who can I contact if I have questions?
If you have questions about:
- Insurance status
- Self-pay options
- HCFA forms or documentation
Please contact our front office team — we’re happy to help!
